Revive currently offers services that may include physical therapy, strength training, personal training, running performance services, run coaching, wellness services, and related services.
These Terms primarily govern use of the Website. They do not replace any separate agreement, consent form, financial policy, cancellation policy, coaching agreement, intake document, or other terms that may apply when you become a patient or client of Revive.
If a service-specific agreement conflicts with these Website Terms regarding that service, the service-specific agreement will control.
2. Website Information Is Not Medical Advice
Information presented on the Website is provided for general educational and informational purposes.
Website content is not intended to constitute individualized medical advice, diagnosis, treatment, or a substitute for an evaluation by an appropriately licensed healthcare professional.

3. No Provider-Patient Relationship Through Website Use

Likewise, submitting an inquiry or application for coaching or training does not guarantee acceptance into a Revive service.
4. Emergencies and Urgent Medical Concerns
The Website and Revive's general contact methods are not emergency services and should not be used for urgent or emergency medical needs.
If you are experiencing a medical emergency, call 911 or seek immediate care from an appropriate emergency medical provider.
Do not rely on Website forms, email, text messaging, social media, or other non-emergency Revive communication channels for time-sensitive emergency assistance.
5. Physical Therapy Services
Physical therapy services are provided only within the scope permitted by applicable professional licensing laws, regulations, and standards.
Information provided through the Website about physical therapy is general in nature. The appropriateness of physical therapy, any clinical recommendations, and an individual's plan of care are determined through the applicable professional evaluation and clinical process.

6. Strength Training, Personal Training, and Wellness Services
Strength training, personal training, wellness, and fitness services are designed to support fitness, strength, performance, and related goals.
Participation in exercise and physical activity involves inherent risks, including the possibility of soreness, strains, falls, aggravation of existing conditions, and other injury.
Specific participation requirements, informed-consent provisions, assumption-of-risk terms, and other conditions applicable to these services may be addressed separately before or during enrollment.
Nothing on the Website guarantees that a particular exercise program or service is appropriate for every individual.
7. Run Coaching and Remote Services
Revive may provide run coaching and related remote services to eligible clients located within the United States.
Run coaching is intended to provide individualized training guidance, programming, education, and performance support. Unless expressly provided as part of a separately established healthcare relationship, run coaching is not physical therapy, medical care, diagnosis, or treatment.
Clients are responsible for communicating relevant limitations, symptoms, injuries, medical restrictions, and material changes that could affect their ability to participate safely.
Revive may recommend that a client seek medical evaluation or other professional care when circumstances reasonably warrant it.
Availability of remote services may vary depending on the service requested, the client's location, professional licensure requirements, and other applicable legal or professional requirements.
8. Minors
Individuals under 18 years of age should not independently submit Website forms, applications, or personal information to Revive.
A parent, legal guardian, or other legally authorized representative must submit inquiries and applications on behalf of a minor and must complete any required consent or enrollment process for services provided to that minor.
Additional requirements may apply to healthcare services provided to minors under applicable law.
9. Service Availability, Pricing, and Website Information
We make reasonable efforts to keep information on the Website accurate and current. However, services, scheduling availability, pricing, programs, personnel, descriptions, policies, and other Website information may change.
Website information does not constitute a guarantee that a particular service, appointment, program, price, or time slot will remain available.
Current fees, payment obligations, cancellation policies, package terms, refund provisions, and other financial terms applicable to a particular service may be provided separately when you schedule or enroll.

Revive does not guarantee that use of any service will eliminate pain, prevent injury, produce a particular performance improvement, achieve a particular race result, or result in any other specific outcome.
Any examples, case descriptions, testimonials, or client experiences presented on the Website reflect the circumstances of the individuals involved and should not be interpreted as a guarantee that another person will experience the same outcome.
